Koshland Pharm: Custom Compounding Pharmacy is an innovative community pharmacy. We make medications tailored to a patient's specific needs. For example, sometimes a patient could benefit from a pain medication in a cream rather than capsule form or from a medication free of allergens such as lactose or gluten. To address this kind of specialized need, we make high-quality, customized prescriptions.
Our mission is to improve the health and well-being of our patients by building strong relationships between pharmacists, patients and doctors and by serving as a knowledgeable resource for healthy living, aging and well-being.
We are the only compounding pharmacy in San Francisco accredited by the Pharmacy Compounding Accreditation Board (PCAB), demonstrating our commitment to safety and quality.

| PETER KOSHLAND'S BIO: |

Peter Koshland (Pharm.D),founder of Koshland Pharm, is a graduate of the UCSF School of Pharmacy, where he is currently an Adjunct Professor of Clinical Pharmacy and a member of the Alumni Association's Board of Directors. Before starting Koshland Pharm, he was the Director of Pharmacy and Compounding for Elephant Pharm, a Bay Area pharmacy chain that merged eastern and western medicine. Like many Bay Area residents, he lost his job during the economic downturn. Peter took the opportunity of being out of work to launch his life-long dream of opening an independent pharmacy, Koshland Pham, in 2009.
As a pharmacist, Peter is committed to supporting the health and well-being of his patients through close collaboration with practitioners in the application of evidence-based medicine. He has extensive experience in helping patients improve their health by addressing issues of chronic stress, optimizing thyroid function and using bio-identical hormone replacement therapy.
An experienced speaker, Peter has given presentations for diverse audiences, including the California Naturopathic Doctors Association, UCSF's Annual Integrative Medicine Network's Forum, and UCSF's School of Pharmacy. He has recently received national and state recognition as a compounding pharmacist, being named the Pharmacist of the Month (January, 2012) by the Professional Compounding Centers of America (PCCA), being appointed to PCCA's 2012 Advisory Council, and being elected as a Board Member of the California Pharmacists Association Academy of Compounding Pharmacists (2012-2013). |

| KOSHLAND PHARM'S STAFF: |

| The staff of Koshland Pharm includes two pharmacists, one graduate intern pharmacist, three pharmacy technicians, and one marketing and outreach director. Two pharmacy student interns also spend six week rotations at Koshland Pharm, where they learn the skill of making customized prescription drugs. Mary Anne Koda-Kimble, Pharm.D, Dean of the UCSF School of Pharmacy from 1998 to 2012, has said of Koshland Pharm, "Thank you, Koshland, for your commitment to pharmacy education and to the advancement of our profession." |
