Prescriptions should include: |

- Patient name
- Patient phone number
- Medication
- Dosage form (e.g. cream, ointment, capsule, troche, etc.)
- Medication strength
- Quantity
- Directions for use
- Prescriber’s name
- Prescriber’s license and DEA number (if new to us)
- Number of refills
- Any special instructions to include with the prescription
